What does 999.9 mean on a gold bar?

It means 999.9 parts of gold per thousand, or 99.99% fine gold. The bar’s weight, refiner, serialisation and documentation can also matter.

Why is a physical bar price different from spot?

Spot is a market reference. A physical quote can also reflect fabrication, logistics, availability, financing, verification and the dealer bid/offer spread.
